If you wish to be audtioned, kindly ensure that you have returned:
  1. Application Form with ALL sections including medical questionnaire completed.

  2. Passport style photograph of yourself to be attached to application form.

  3. Audition fee of €30.00
    (Please make cheque / postal order payable to College of Dance)
